VAT Relief Form

To complete the VAT relief process, an Eligibility Declaration Form must be completed by MattressOnline and the customer. 

Here are the steps you need to follow to claim back your VAT on a product purchase: 

1. Please fill in ‘Part 2. Customer's declaration’ Eligibility Declaration Form PDF.
2. Send the completed form back to Customer Service at customerservice@mattressonline.co.uk. 
3. We will then fill in the ‘Part 1. Supplier’ section of the PDF if the products we are supplying qualify.
4. We keep this form for our VAT records.
5. The refund will be processed onto the card used for the original payment within 14 days.
